Payment Options for Senior Care

Care Types & How They Are Paid For

Independent Living

What it is:
A maintenance-free lifestyle for active seniors with little or no personal care.

How it’s paid:

  • Private pay only

  • LTC insurance rarely covers

  • VA Aid & Attendance rarely covers

  • Medicare/Medicaid do not cover

Residential Home Care / Home Health

What it is:
Caregivers provide help with ADLs (bathing, dressing, meals) in the home.

How it’s paid:

  • Mostly private pay

  • Medicaid waivers may help (varies by state)

  • Some LTC insurance covers home care

  • Medicare only covers short-term medically necessary home health after illness/injury—not ongoing custodial care

Assisted Living

What it is:
Housing with personal care, meals, medication support, and activities.

How it’s paid:

  • Primarily private pay

  • Some LTC insurance

  • Some state Medicaid waivers (state-specific)

  • VA Aid & Attendance may contribute

  • Medicare does not cover room & board

Memory Care

What it is:
Specialized assisted living for dementia/Alzheimer’s with secure environments and trained staff.
Costs are typically 15–30% higher than standard assisted living.

How it’s paid:

  • Mainly private pay

  • LTC insurance may help

  • VA Aid & Attendance

  • Some Medicaid waivers

  • Medicare does not cover custodial care

Payment Options Explained

Private Pay

  • Most common method across all care types

  • Includes personal savings, retirement income, home sale proceeds

  • Many seniors “spend down” to eventually qualify for Medicaid

  • Paid directly to the facility or provider

Community-Based Organizations (CBOs)

  • Provide free or low-cost support (adult day care, transport, meals)

  • Do not cover residential care costs

  • Services vary widely by location

Long-Term Care Insurance (LTCI)

  • May cover home care, assisted living, or nursing homes

  • Coverage depends entirely on the policy

  • Pays a daily/monthly benefit amount

  • Starts after meeting eligibility + elimination period

VA Aid & Attendance (A&A)

  • For eligible veterans or surviving spouses needing help with ADLs

  • Can be used for home care, AL, or memory care

  • Paid monthly directly to the veteran/spouse

Medicaid

  • Largest public funder of long-term care

  • For low-income individuals only (needs-based)

Covers:

  • Nursing homes (all states)

  • Some home and community-based services

  • Some states cover assisted living through waivers

Does NOT cover:

  • Independent living

Medicaid pays the provider directly once eligibility is met.

Medicare

  • For seniors 65+

  • Covers short-term skilled care only

Covers:

  • Up to 100 days in skilled nursing after a qualifying hospital stay

  • Medically necessary home health

Does NOT cover:

  • Assisted living

  • Memory care

  • Independent living

  • Long-term custodial care.
